RESUMO

ObjectiveMalignant tumor has become a serious public health problem, threatening human health. Chinese herbal medicine has become a research focus as it is "simple, convenient, inexpensive, and effective" and exerts anti-tumor effect through "multiple components, multiple targets, and multiple pathways". This paper reviews the anti-tumor effect of triterpenes, polysaccharides and other compounds in Poria, which is expected to serve as a reference for research on the anti-tumor effect of Poria. MethodBasic research, clinical observation, and reviews were retrieved from PubMed, Google Scholar, and China National Knowledge Infrastructure (CNKI) (1995—2022) with the keywords of Poria and malignant tumor. ResultThe main active components of Poria such as triterpenes represented by pachymic acid and polysaccharides represented by pachyman had extensive pharmacological effects in inhibiting the proliferation of malignant tumors, inducing apoptosis, suppressing invasion and metastasis, regulating the immune function, and improving the quality of life. In addition, the cultivation method, medicinal part, growth environment, and harvest cycle of the medicinal plant, and dosage form and processing method of Poria affected the content of different active components. ConclusionThe active components of pachymic acid and triterpenoids have great potential in antitumor, which, however, face the challenges in innovation and quality clinical trials. The lack of quality control standard of Poria may affect the anti-tumor effect, and efforts should be made to formulate specific standard for quality control. The antitumor of Poria is science-based and thus it has broad application prospect. Nevertheless, more efforts should be made before the practical application.

ObjectiveTo investigate the effect and underlying molecular mechanism of astragaloside-Ⅳ (AS-Ⅳ) on autophagy and apoptosis of nasopharyngeal carcinoma cells. MethodIn experiments in vitro, the effect of AS-Ⅳ on the autophagy of nasopharyngeal carcinoma cells was observed by monodansylcadaverine (MDC) staining and transmission electron microscopy (TEM). In experiments in vivo, immunofluorescence (IF) and Western blot were used to detect the changes in autophagy and apoptosis and the expression of key proteins in the phosphatidylinositol 3-kinase/protein kinase B/mammalian target of rapamycin (PI3K/Akt/mTOR) signaling pathway after the establishment of a xenograft tumor model in nude mice. ResultAfter 5-8F cells were treated with AS-Ⅳ of different doses (5, 10, 20 μmol·L-1), the fluorescence intensity of autophagy in AS-Ⅳ groups significantly increased as compared with that in the blank group. The fluorescence expression of autophagy in AS-Ⅳ groups was the strongest after intervention for 24 hours, and the fluorescence expression in the 10 μmol·L-1 AS-Ⅳ group was the most obvious. The autophagy activator rapamycin (RAPA) induced more autophagosomes in 5-8F cells under the transmission electron microscope, and 3-methyladenine (3-MA), an autophagy inhibitor, did not induce autophagosome formation in 5-8F cells under the transmission electron microscope as compared with the results in the blank group. In the 10 μmol·L-1 AS-Ⅳ group, the intracellular structure and cell membrane were intact and clear, and autophagosome formation was observed. Compared with the blank group, the AS-Ⅳ groups showed inhibited tumor volume (P<0.05, P<0.01), potentiated fluorescence signals of microtubule-associated protein l light chain 3 type Ⅱ/microtubule-associated protein l light chain 3 type Ⅰ (LC3 Ⅱ/Ⅰ) and cleaved Caspase-3 (P<0.05, P<0.01), increased expression levels of the mammalian homolog of yeast ATG6 (Beclin-1), LC3 Ⅱ/Ⅰ, cleaved Caspase-3, and cleaved PARP (P<0.05, P<0.01), down-regulated expression of ubiquitin-binding protein (p62) (P<0.05, P<0.01), and reduced protein expression levels of phosphorylated PI3K (p-PI3K), phosphorylated Akt (p-Akt), and phosphorylated mTOR (p-mTOR) (P<0.05, P<0.01). ConclusionAS-Ⅳ can induce autophagy and apoptosis of nasopharyngeal carcinoma cells, and the mechanism is presumably attributed to the activation of the PI3K/Akt/mTOR signaling pathway.

OBJECTIVES@#To observe the electrophysiological changes of astrocytes in the process of hyperoxia induced apoptosis and analyze the relationship between electrophysiological characteristics and morphological changes.@*METHODS@#Astrocytes were exposed to 90% hyperoxia for 12-72 h. The electrophysiological characteristics of astrocytes in each group were detected by patch clamp technique, and the morphological characteristics of astrocytes were observed at the same time. Then the same batch of astrocytes were collected, and the expression levels of caspase-1, caspase-3, gasdermin D (GSDMD) and gasdermin E (GSDME) were detected by Western blotting.@*RESULTS@#From 12 h to 72 h after hyperoxia exposure, the inward current was significantly lower than that of the control group (0.05). At each time point, the morphology of cells changed correspondingly. Western blotting showed that the expression of caspase-1 was increased significantly at 24 h and decreased significantly at 72 h after hyperoxia exposure (0.05), but began to decrease at 48 h (<0.05); GSDME increased gradually at 24 h after hyperoxia exposure (<0.05).@*CONCLUSIONS@#Under hyperoxia exposure, the ion channels of astrocytes are damaged, which can maintain the dysfunction of ion homeostasis, activate GSDME, induce the damaged cells to break away from the apoptotic pathway, and mediate the pyroptosis.

RESUMO

Nasopharyngeal carcinoma (NPC), a common head and neck neoplasm in southern China,is mostly treated with radiotherapy, however, it can not be completely cured by chemoradiotherapy. Recent investigation on NPC suggests that the nasopharyngeal carcinoma stem cells can be differentiated to heterogeneous cancer cells that have a strong ability for self renovation and further contribute to the development and progression of tumor itself which is closely related to drug resistance, recurrence and metastasis of NPC. Wnt/β-catenin, Notch, Hedgehog, NF-κB and mTOR signaling pathways play important roles in cancer stem cells. Study on nasopharyngeal carcinoma stem cells targeted therapies and its related pathways provides a new strategy for the clinical treatment of NPC.

Objective To improve the cleaning quality of reusable medical instruments continuously through root cause analysis,reduce healthcare-associated infection(HAI)and the wear and tear of instruments.Methods The major factors influencing the cleaning quality of instruments were analyzed and clarified by fishbone diagram,contin-uous quality improvement was carried out based on 5 terminal factors,the rate of rusting,stain,repeated cleaning, damage,and clinical department satisfaction before and after the implementation of continuous improvement were compared.Results The qualified rate of instrument clean increased from 94.24% before implementing measures to 97.60% after implementing measures,the rate of rusting,stain,repeated cleaning,damage decreased from 3.39%, 2.37%,5.76%,and 2.08% to 1 .55%,0.85%,2.40%,and 0.48% respectively,there were significant difference among different groups(all P <0.001 );the score of satisfaction rate to central sterile supply department elevated from(93.87±3.87)before implementing measures to (98.08 ±0.59)after implementing measures(t =6.80,P <0.001).Conclusion Root cause analysis is important in improving cleaning quality of reusable instruments continu-ously,as well as reducing HAI and the wear and tear of instrument,it can save medical cost.

RESUMO

With the wide application of electronic data management (EDC), the data management is shifting to a new mode. In order to recognize the advantages of EDC, we choose 20 representative registered clinical trials, which involve 5 404 subjects and 321 sites. We found that EDC has many beneficial impacts on the course of clinical trial data management, including the process of data collection, data cleaning, data quality control and clinical trial decision-making. The result also provides a reference for the adoption of EDC in clinical trials.

RESUMO

Electronic case report forms (eCRFs) instead of the traditional paper case report forms (pCRFs) are increasingly used by investigators and sponsors of clinical research. We include a total of 14 phase III studies (8 pCRF, 6 eCRF) to compare paper and electronic data documentation both quantitatively and qualitatively in clinical studies. The result suggests that adaptions of electronic data capture (EDC) in clinical trials have the advantages in optimization of data capture process, improvement of data quality and earlier clinical decision compared to paper-based methods. Furthermore, the successful implementation of EDC requires accouplements with corresponding data management processes and reallocation of resources.

Due to a great amount of data in clinical trials, the data cleansing needs to adopt a variety of measures, including the latest developed visual check approach. According to the different types of clinical data and the different stages in the course of clinical data management, this study reviews 8 types of visual graphics that show the relevance and trend among the data. The series of graphics can rapidly detect abnormal data, monitor clinical research in real-time, make the data management process much easier and improve the clinical trial efficiency and data quality.

RESUMO

Objective To study the cell metabolism changes of hippocampal region in patients with vascular dementia (VD) after stroke by 1 H-magnetic resonance spectroscopy (1 H-MRS),and to observe the relationship between cell metabolism changes and clinical symptoms after donepezil hydrochloride treatment.Methods Enrolled 18 patients with VD after stroke,16 males and 2 females,with a mean age of (76.9±2.7) years,13 patients without VD after stroke,11 males and 2 females,with a mean age of (76.2±3.6) years and 14 health volunteers as control group,12 males and 2 females,with a mean age of (75.1+2.4) years.The mini-mental state examination (MMSE),activities of daily living (ADL) and Hamilton depression scale (HAMD) were used to assess the effects on cognitive function,ability of daily living and behavior symptoms.1 H-MRS examination of bilateral hippocampus was conducted in all subjects.Metabolisms of N-acetylaspartate (NAA),choline (Cho) and creatine (Cr) were examined and the ratios of NAA/Cr and Cho/Cr were calculated.The patients with VD were prescribed donepezil and given 1 H-MRS assessment half a year later.Results The patients with VD had a significantly lower NAA/Cr ratio in bilateral temporal hippocampus area than did health controls and the patients without VD (F=4.23,4.98,both P<0.05),and a higher Cho/Cr ratio than did health controls and the patients without VD in left hippocampus (P<0.05).The NAA/Cr and Cho/Cr ratios in patients without VD had no significant changes compared to health controls.There was a positive relationship between MMSE score and NAA/Cr ratio,and negative relationship between MMSE score and Cho/Cr ratio in the bilateral hippocampus area in patients with VD.After 6 months of treatment of donepezil hydrochloride,the MMSE and ADL scales significantly increased (both P<0.05),and NAA/Cr ratio had no significant change in hippocampal region (t=-2.02,-2.04,both P>0.05),whereas Cho/Cr had significant decrease (t=2.86,2.23,both P<0.05).Conclusions The data suggest that the lower NAA/Cr ratio and higher Cho/Cr ratio in hippocampus area might be helpful in distinguishing VD and no-VD in patients with stroke.Donepezil improves general cognition function in patients with VD,which may be associated with regulating the cholinergic pathogenesis of VD in vivo and in vitro.

This study aims to save cost of sampling for estimating the area under the amlodipine plasma concentration versus time curve in 24 hours (AUC(0-24 h)). Limited sampling strategy (LSS) models was developed and validated by mutiple regression model within 4 or fewer amlodipine concentration values. Absolute prediction error (APE), root of mean square error (RMSE) and visual predict check were used as criterion. The results of Jackknife validation showed that fifteen (9.4%) of the 160 LSS based on regression analysis were not within an APE of 15% by using one concentration-time point. 156 (97.5%), 159 (99.4%) and 160 (100%) of the 160 LSS model were capable of predicting within an APE 15% by using 2, 3, 4 points, separately. Limited sampling strategies have been developed and validated for estimating AUC(0-24 h) of amlodipine. The present study indicated that the implemention of both 5 mg and 10 mg dosage could enable accurate predictions of AUC(0-24 h) by the same LSS model. This study shows that 12, 4, 24, 2 h after administration are key sampling time points. The combination of (12, 4), (12, 4, 24) or (12, 4, 24, 2 h) might be chosen as sampling hours for predicting AUC(0-24 h) in practical application according to requirement.

OBJECTIVE@#To investigate the effect of several traditional Chinese medicine formula therapies on proliferation of nasopharyngeal carcinoma cell line HNE1.@*METHOD@#The inhibition effect of medicine serums of Qi-Boosting Toxin-Resolving granule on proliferation of NPC cell line HNE1 in vitro was observed by MTT assay.@*RESULT@#The rates of survival cells in Qi-Boosting Toxin-Resolving decoction group and Toxin-Resolving group were the lowest after 48 hours treatment, while Qi-Boosting group, yin-nourishing group, residual-component group medicine serums had the most obvious effects on the rates of survival cells at 72 hours. The inhibition ratio of 15% Qi-Boosting Toxin-Resolving granule medicine serums was 67.68%.@*CONCLUSION@#The result of MTT assay showed that cell proliferation was significantly inhibited by Qi-Boosting Toxin-Resolving granule and its separate composes medicine serums. Qi-Boosting Toxin-Resolving granule had the most significantly specific effects. The effects were correlated with serum concentration and treatment time of Qi-Boosting Toxin-Resolving granule and its separate composes medicine.

OBJECTIVE To investigate the effects of N,N'-Dinitrosopiperazine (DNP) on the cell proliferation of nasal and nasopharyngeal epithelia and the expression levels of TRAF2 and p16 genes in TgN (p53mt-LMP1)/HT transgenic mice and the relationships between them.METHODS The epithelial proliferating characteristics of nasal cavity and nasopharynx in TgN (p53mt-LMP1)/HT cancerous lesion inducing group treated by DNP (TI),TgN (p53mt-LMP1)/HT controlling group (TC), C57BL/6J cancerous lesion inducing group (CI) treated by DNP and C57BL/6J controlling group (CC) were observed for pathological evaluation by HE staining,and the expression levels of TRAF2 and p16 genes in these tissue samples were determined by immunohistochemical methods.RESULTS The occurring rates of precancerous lesions in nasal and/or nasopharyngeal epithelia in TI,TC,CI and CC groups were 90%,10%,0 and 0,respectively (P

Background and purpose:The development of nasopharyngeal carcinoma in rat and transformation of embryo nasophryngeal epithelia induced by N,N'-dinitrosopiperazine (DNP) were interfered by Yiqijiedu formular medicine via inhibiting telomerase. We investigated the impact of Yiqijiedu formula on precancerous lesion of nasal and nasopharyngeal epithelia in TgN (p53mt-LMP1) / HT mice induced by DNP and on its expression level of c-jun and p16 genes. Methods:The fourth generation TgN (p53mt-LMP1) / HT mice aged 5 months were randomly divided into 2 groups:(1)TgN(p53mt-LMP1)/HT cancerous lesion-inducing group (group A) was treated by normal saline with 15ml/kg, and Chinese traditional medicine treating group (group B) was treated by Yiqijiedu formular drug with 12.91 g/(kg?d). C57BL/6J wild-type mice were chosen as negative control group (group C) treated by normal saline with 15ml/kg. Epithelial tissue samples were taken from nasal cavity and nasopharyngnx of mice respectively for pathohistological evaluation by H-E staining procedures, and expression level of c-Jun and p16 in these tissue sampleswere determined by immunohistochemical methods and western blot. Objective: To investigate the biological phenotype and cell cycle distribution characteristics of nasal and/or nasopharyngeal epithelia of TgN(p53mt-LMP1)/HT mice.Methods: The biological phenotype changes(mainly the incidence of precancerous lesion) in nasal and nasopharyngeal epithelial tissues of the generation G4 of TgN(p53mt-LMP1)/HT mice aged 5,11,15 and 18 months were determined by H-E staining,and the cell cycle characteristics of nasal and nasopharyngeal epithelia detected by flow cytometry(FCM).Results: The incidences of precancerous lesion in the nasal and nasopharyngeal epithelia were 0,50%,60% and 75% respectively in the 5,11,15 and 18 mos groups of the positively expressed transgenic mice,and 0 in the four age groups of the negatively expressed ones.Compared with the negatively expressed transgenic mice,the number of nasal or nasopharyngeal epithelial cells was markedly decreased in the G0/G1 phase,but obviously increased in the S and G2/M phases,with the proliferation index(PI) significantly enhanced(P

Epstein-Barr virus is a kind of herpes virus that has oncogenic potential and is related to the tumorgenesis process of various malignancy. Important advances have been obtained on the study of the structure and functions of Epstein-Barr virus(EBV) latent membrane protein 1(LMP1) and signal transduction pathways mediated by LMP1 in recent years. LMP1 could regulate cellular growth, proliferation, transformation, differentiation, transference and apoptosis by means of signal transduction pathways such as NF-?B, AP-1, JAK/STAT, Ets1, MAPK/ATF2 and cyclin dependent kinase.

RESUMO

Researches have indicated that matrix metalloproteinases (MMP) and vascular endothelial growth factors (VEGF) play a key role in the metastasis of nasopharynx carcinoma (NPC). They promote tumor cell infiltration and metastasis by proteolysis of extracellular matrix proteolysis of tumor cells and induction of angiogenesis and lymphangioenesis. Researches on the mechanism of MMPs and VEGFs on NPC may help to further understand the metastasis of NPC and develop new methods for the diagnosis and treatment of NPC and other tumors as well.

As one of the "three key virtues" of Confucianism,wisdom played a significant role in the thousands of years of traditional Chinese culture.The nature of medical skills is humanity.However,as the great development of medical technology in modern society,tension and conflicts also arise in the current physician-patient relationship.Under this circumstance,this paper explores the origin of Confucian wisdom and its connection with modern physician-patient relationship,and comes up with suggestions for constructing a modern harmonious physician-patient relationship.
